The rheumatology questionnaire is a mixed-reality application developed in-house for the paediatric ward for reducing anxiety in adolescents. Patients often come in and are asked to fill out questionnaires while waiting before their appointments. However, the staff find that patients are particularly anxious and do not engage well with completing clipboard questionnaires.

To help with this, this project serves as a novel solution that aims to help distract and better engage users in answering questions. The user puts on a headset, and is greeted by a virtual-worm mascot that guides them through a range of different minigames for answering questions about their health. At the end, the user can submit their answers virtually into a post-box, which posts their answers anonymously to a spreadsheet for the clinicians to view.
